FINANCIAL AID APPLICATION

Our mission is to reduce the flow of dogs into the shelter system. This includes assisting with financial aid for those who cannot afford life-saving surgeries and/or procedures that can result in surrendering a pet. If you are in need of financial aid please fill out the form below.

We are unable to provide financial assistance at Shively Animal Clinic. If your pet was seen at Shively, we ask that you get a second opinion.
